How to sign in

Use your provider account whether you work with a clinic or run your own practice.

  1. Enter your email and password

    Open CoCharts Mental Health and use the email tied to your provider account. If your clinic invited you, accept that invitation with the same email first.

  2. Continue to the provider portal

    CoCharts sends both clinic-affiliated clinicians and solo providers to the provider portal. Complete any setup steps shown before opening Home.

  3. Recover access if needed

    Choose Forgot password on the sign-in page to request a reset link. If a clinic invitation has expired, ask your clinic administrator to resend it.
